QR entry, clean final URL, reversible channel control

This gate sits in front of your report. The QR code points to /r/<token>, the server validates the token and optional password, then issues a short session and redirects to a clean address.

If someone copies the final address bar, they only copy /p/<pageId>. Access still depends on a valid session cookie.
